Enrollment
For your convenience we have all enrollment information available online. To enroll your child at Sahuarita Intermediate School, please make sure to bring in all enrollment paperwork below:
- Enrollment Forms (English)(Spanish)
- Student Medical Information
- Home Language Survey (English)(Spanish)
- Records Request
- Open Enrollment Form
- Transportation Compact
Along with these forms we need the following:
- Birth Certificate (original)
- Immunization Records
- Proof of Residency: utility bill (gas or electric) or copy of lease/mortgage. PO Boxes are not accepted without a physical address
- Copy of a parent/guardian ID (ID will not count as Proof of Residency)
- Guardianship papers (if applies)
