SUSD is partnering with SPARCC and Athlon Physical Therapy for the 2019 SUSD Sports Physical Event.

at the SHS GYM

$10 per student physical. Cash or checks payable to SUSD#30.

Save some time! Download the physical form and fill out the parent/guardian section (page 1&2) prior to the event.

Open to all SUSD middle and high school athletes, ages 11-18. All minors must be accompanied by a parent or legal guardian.

When you arrive:

1. If you have not completed the parent/guardian section of the AIA physical form prior to your arrival, please request a form and complete the information on the first 2 pages. 
2. Please check in and make your payment.
3. The SUSD student-athlete will move through various stations in the SHS gym to complete the physical form. 
4. Once your child has completed all stations, you are free to leave.
