COVID Vaccine Registration – Ages 16 and up

All Arizona residents ages 16 and older are now eligible to register for the COVID-19 vaccine through the AZDHS State COVID-19 Vaccination Pods. 
 
Adults ages 18 and up are eligible to receive the Pfizer, Moderna, or Johnson and Johnson Vaccines.
 
Minors who are age 16 or 17 years old are only eligible to receive the Pfizer vaccine and must be accompanied by a consenting parent or legal guardian in order to receive the vaccine. Appointments for the Pfizer vaccine can be made at the State of Arizona Vaccination Pods at either the University of Arizona in Tucson or the State Farm Stadium in Glendale.
 
ALL individuals (and parents/guardians of minors) are required to present identification to verify identity and age, such as a driver’s license or state ID, at the time of their visit.
