SUBJECT: EMPLOYEE NOTICE OF CONFLICT OF INTEREST

According to A.R.S. §38-503 and Governing Board Policy GBEAA, any employee who has, or whose relative has, a substantial financial interest in any contract, sale or purchase with the district must make this interest known to the district and refrain from participating, in any manner as an employee of the district, in any decision or deliberations about such contract, sale or purchase. Therefore, I am submitting this memo to fully disclose the following potential conflict of interest.

I have removed myself from the decision-making process as an employee of the district relating to the potential contract, sale or purchase with the district. Please maintain this disclosure in the district’s official records.
